Parents will need either their student’s ID number and birthday or their student’s first/last name, birthday, and last 4 of SSN to make a transportation request. Transportation requests must be submitted by 7/28 to be guaranteed FDOS transportation service, though we will continue to accept requests after that date. Remember, this year campuses will not be requesting transportation for students; the request must come from parents directly via this survey.
